Clinical Pharmacy Specialist Pharmacy Intern (Job Overview)

Schedule: 8-12 Hours Per Week

Key Responsibilities
  • Provide pharmaceutical care services through direct patient and provider interactions
  • Conduct chronic disease state medication management services via face-to-face or virtual care
  • Obtain comprehensive medical and medication histories to ensure safe and effective care
  • Reconcile patient medications during ambulatory care clinic visits
  • Perform physical assessments and triage patients as needed
  • Document encounters and interventions in the electronic medical record
  • Design, implement, and monitor therapeutic plans under your scope of practice
  • Initiate, modify, renew, refill, or discontinue medication therapy
  • Order labs and tests to determine the efficacy and toxicity of medication therapy
  • Document and report adverse drug events and medication allergies
  • Provide drug information and education to patients, families, and providers
  • Assess medication adherence during counseling sessions
  • Set priorities and work independently while collaborating with others
  • Serve as a preceptor to pharmacy students and residents
  • Perform other duties as assigned
Minimum Qualifications
  • Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
  • Completion of ASHP accredited PGY-1 Pharmacy Residency
  • Valid license from the Board of Pharmacy of the State of Texas
  • Certification in Basic Life Support / First Aid / CPR
  • 2 years of medication adherence counseling experience
  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics is preferred
  • Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics is preferred
  • Certification in immunization delivery or smoking cessation is preferred
  • Computer experience with Windows, Microsoft Word & Excel
  • Must be able to manage multiple deadlines and prioritize
  • Excellent communication skills and customer service skills
