Key Responsibilities- Provide pharmaceutical care services through direct patient and provider interactions
- Conduct chronic disease state medication management services via face-to-face or virtual care
- Obtain comprehensive medical and medication histories to ensure safe and effective care
- Reconcile patient medications during ambulatory care clinic visits
- Perform physical assessments and triage patients as needed
- Document encounters and interventions in the electronic medical record
- Design, implement, and monitor therapeutic plans under your scope of practice
- Initiate, modify, renew, refill, or discontinue medication therapy
- Order labs and tests to determine the efficacy and toxicity of medication therapy
- Document and report adverse drug events and medication allergies
- Provide drug information and education to patients, families, and providers
- Assess medication adherence during counseling sessions
- Set priorities and work independently while collaborating with others
- Serve as a preceptor to pharmacy students and residents
- Perform other duties as assigned
- Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
- Completion of ASHP accredited PGY-1 Pharmacy Residency
- Valid license from the Board of Pharmacy of the State of Texas
- Certification in Basic Life Support / First Aid / CPR
- 2 years of medication adherence counseling experience
- Experience in HIV/AIDs, Geriatrics, and/or Pediatrics is preferred
- Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics is preferred
- Certification in immunization delivery or smoking cessation is preferred
- Computer experience with Windows, Microsoft Word & Excel
- Must be able to manage multiple deadlines and prioritize
- Excellent communication skills and customer service skills
