License/Certification Requirements: Valid MN Driver's License, current insurance or be insurable. Current, unrestricted Registered Nurse (RN) licensure in MN. CPR/BLS.

Summary: Provides high quality, culturally competent nursing care across multiple service area, including Tribal Health Clinics, the Maternal Child Health Program, the Diabetes Center, and Home Health Care. This position offers flexible clinical support where staffing and patient needs are greatest, ensuring continuity of care and adherence to organizational standards. Must demonstrate strong clinical judgement, adaptability, and excellent communication skills while working collaboratively with interdisciplinary teams. Travel between service locations may be required.

Education Requirements and Experience:

Graduate of an accredited Nursing Program (Associate Degree in Nursing).

Bachelor of Science in Nursing (BSN) preferred.

Must maintain all continuing requirements per state licensing guidelines.

Current, unrestricted Registered Nurse (RN) licensure in MN.

Must have or be able to pass clearance for use of the Electronic Health Record in Indian Health Service areas.

Minimum 1 year of clinical nursing experience in a hospital, outpatient clinic, public health, or similar setting.

Experience in primary care, maternal child health, diabetes/chronic disease management, and home health care preferred.

Experience working with tribal communities, community health or culturally diverse populations.

Additional Qualifications and Skills:

Strong background in patient teaching, chronic disease self-management support, and care coordination.

Strong clinical assessment, critical thinking, and problem-solving abilities.

Ability to triage effectively and respond appropriately in urgent or rapidly changing situations.

Competence in medication administration, infection control, wound care, chronic disease monitoring, and patient safety standards.

Strong documentation skills with accuracy, timelines, and compliance with regulatory standards.

Must understand computer applications (Microsoft Word, Excel, Outlook, EHR/RPMS.

Excellent verbal, written, and interpersonal communication skills.

Knowledge of surrounding service areas.

Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe safety sensitive positions.

Duties/Responsibilities:

Deliver patient-centered nursing care according to established policies, procedures, and clinical guidelines in each assigned service area.

Perform assessments, develop nursing care of plans, administer medications, and provide clinical treatments consistent with scope

Support specialty care needs in:

Tribal Health Clinics: outpatient care, triage, chronic disease management, immunizations, patient education.

Maternal Child Health: prenatal/postpartum support, breastfeeding education, newborn assessments, family health education.

Diabetes Center: diabetes education, medication management, monitoring of labs and A1c trends, patient coaching.

Home Health Care: in home assessments, wound care, medication reconciliation, monitoring chronic conditions, patient/family teaching.

Work closely with physicians, advanced practice providers, public health staff, and interdisciplinary care teams to coordinate high-quality patient care.

Assist with care transitions, follow-up visits, and referrals to internal and external programs.

Attend and or conduct all mandatory meetings and trainings as required.

Maintain confidentiality and professional ethics at all times.

Travel in and out of the state on a as needed basis.

Understanding and sensitivity to Native American culture.

Adhere to policies governing staff conduct.

Physical Requirements:

Ability to lift up to 30lbs, assist with patient mobility and perform prolonged standing, walking, or bending

Comfortable working in both clinic and home settings.

Potential for exposure to infectious diseases, bloodborne pathogens, or environmental hazards.
