- Act as the main point of contact for patients and their families regarding all aspects of care received at LaSante.
- Monitor and manage a designated panel of patients, particularly those with chronic health conditions.
- Support patients in understanding and managing their medical conditions, medications, and treatment plans.
- Enroll patients in medical and support programs that meet their needs.
- Facilitate communication between patients, families, and healthcare providers to ensure coordinated, high-quality care.
- Assist patients with scheduling and navigating appointments, referrals, and follow-up care.
- Connect patients to community resources and social support that promote overall well-being.
- Perform additional duties as assigned by supervisors.
- Strong customer service skills
- Proficiency with computers and ability to learn healthcare systems quickly
- Excellent organizational and multitasking skills
- Prior experience in care coordination or case management preferred
