Responsibilities:
LaSante Health Center is seeking a Part-time Cardiologist to provide high-quality, patient-centered cardiovascular care in a federally qualified health center (FQHC) setting one day/week. This role involves evaluating, diagnosing, and managing cardiovascular conditions while collaborating with primary care providers to support comprehensive care for an underserved community.
Responsibilities:
- Evaluate, diagnose, and treat patients with cardiovascular conditions
- Perform cardiovascular assessments and interpret diagnostic tests (e.g., EKGs, echocardiograms, stress tests as applicable)
- Develop and manage treatment plans for acute and chronic cardiac conditions
- Provide patient education on heart health, disease prevention, and medication management
- Collaborate with primary care providers and care teams to ensure coordinated care
- Document all encounters accurately in the electronic health record (EHR)
- Participate in quality improvement and clinical meetings as requested
- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Cardiology
- Current New York State medical license (or eligibility for licensure)
- Experience in outpatient or community health settings preferred
- Strong communication and patient engagement skills
