Primary Care Physician (MD/DO)

We are actively seeking a Board Certified/Board Eligible Primary Care Physician to join a collaborative, value-based outpatient care team in Montgomery, AL. Key Responsibilities
- Provide comprehensive outpatient primary care services
- Manage chronic and preventive care for adult and geriatric patients
- Coordinate specialty referrals, home health, and follow-up care
- Collaborate closely with an integrated team-based care model
- Maintain timely and accurate EMR documentation
Qualifications
- MD or DO from an accredited program
- Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active or eligible Alabama medical license
- Active DEA preferred
- Experience with Medicare Advantage or value-based care preferred
