Nurse Practitioner / Physician Assistant – Skilled Nursing & Long-Term Care
Location: Eugene, OR
We are seeking a compassionate Nurse Practitioner (NP/ARNP) or Physician Assistant (PA) to provide medical care in Skilled Nursing and Long-Term Care settings.
Responsibilities
- Assess, diagnose, and treat geriatric and post-acute patients
- Develop and manage treatment plans
- Prescribe medications and interpret labs/tests
- Provide post-hospital follow-up
- Document care in the EMR
- Collaborate with physicians and interdisciplinary teams
- Ensure regulatory and quality compliance
Requirements
- Graduate of an accredited NP/ARNP or PA program
- Current Oregon license and national certification
- SNF/LTC, geriatrics, post-acute, or primary care experience preferred
- Strong clinical, communication, and organizational skills

Salary : $90,000 - $115,000
