What are the responsibilities and job description for the Therapeutic Case Manager position at Lamoille County Mental Health Services?
Job Summary:
The Therapeutic Case Manager will provide case management, psychoeducation, and clinical recommendations to a caseload; made up of children and youth being served in the outpatient program up to the age of 22, performing 22-25 hours of direct service per week. The case manager works with families to develop treatment plans and implement therapeutic interventions to support clients in reaching established goals across settings, including in the home, school, and community. Additionally, the case manager is responsible to coordinate services, collaborate with other service providers, and maintain appropriate documentation of services provided.

Education/Experience
Required: Bachelor’s Degree in human services or related field.
