Position Summary
The Service Coordinator provides coordination to children and adults who have developmental disabilities. This includes community/home support, employment, and respite. This coordination occurs in a home community based setting while following Medicaid State and agency guidelines. It is the incumbent’s job to ensure that consumers’ goals are being met.
