Job Summary:

The Wound Care Nurse is responsible for the comprehensive assessment, planning, implementation, and evaluation of care for patients with acute and chronic wounds in the inpatient and swing bed settings, and in the outpatient setting when operationally appropriate under Rural Health Clinic (RHC) guidelines.

This role serves as the clinical expert in wound prevention and management, provides education and training to nursing staff, determines wound care supply needs, and supports development of evidence-based wound care protocols. The Wound Care Nurse collaborates with providers, nursing staff, and interdisciplinary team members to ensure high-quality, cost-effective wound care services and improved patient outcomes.

Work Schedule: Four days per week; Off Wednesday; 8am-4pm; schedule determined based on patient census and operational needs

Qualifications:

· Current Registered Nurse (RN) license, active and in good standing

· Minimum five (5) years of clinical nursing experience

· Current Wound Care Certification (e.g., WOCN, CWCN, CWS, or equivalent) with at least one year of experience

· Current BLS certification

· Demonstrated experience in wound assessment and treatment in acute care, long-term care, or similar setting

· Strong knowledge of pressure injury prevention, staging, and evidence-based wound care practices

· Excellent communication, teaching, and organizational skills\

Preferred:

· Experience in swing bed or rural healthcare setting

· Experience developing policies and clinical protocols

· Experience in outpatient wound care services

Responsibilities include:

· Perform comprehensive wound assessments for inpatient and swing bed patients

· Round regularly on identified wound patients and monitor healing progress

· Develop and implement individualized wound care treatment plans in collaboration with providers

· Provide outpatient wound care services when operationally feasible and compliant with RHC standards

· Ensure accurate, timely documentation of wound assessments, interventions, and outcomes in the electronic medical record

· Determine appropriate wound care supply needs and assist in standardizing wound care products

· Monitor inventory and coordinate ordering of wound-related supplies

· Evaluate product effectiveness and cost-efficiency

· Provide ongoing education and competency training to inpatient and swing bed nursing staff regarding:

o Pressure injury prevention

o Wound staging and documentation

o Dressing selection and application

o Prevention strategies for high-risk patients

· Assist in development and maintenance of wound care policies, protocols, and clinical guidelines

· Track and trend wound-related quality metrics, including hospital-acquired pressure injuries

· Participate in quality improvement initiatives related to wound prevention and management

· Collaborate with leadership to support potential development of outpatient wound care services

· Serve as a clinical resource and consultant for wound-related questions

Physical Requirements for the Role:

· Ability to stand, walk, bend, kneel, and reposition patients as needed

· Ability to lift and assist patients per safe patient handling guidelines

· Manual dexterity is sufficient to perform wound assessments and dressing applications

· Visual acuity sufficient to accurately assess wounds

· Ability to work in a fast-paced clinical environment

Work Environment:

The med/surg unit is a fast-paced, high-acuity environment with high emotional intensity where you will care for patients with diverse, complex, and rapidly changing conditions. The workload can be heavy, so we focus on creating a multi-disciplinary, team-oriented environment to ensure patient centered care. There is the potential for exposure to infectious patients, harmful physical agents, and hazardous or toxic substances.
