As a Case Manager, you’ll make a direct impact on patients’ health journeys by:
- Coordinating care across providers and community resources
- Advocating for patients and families
- Building strong relationships to improve patient outcomes
Key Responsibilities
Case Management
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
- Create care plans and comprehensive assessments for patients, including those with complex needs and Social Determinants of Health (SDOH).
- Communicate patient needs to families, providers, and other care team members.
- Work collaboratively with healthcare teams to develop, implement, and follow up on treatment plans.
- Develop relationships with community resource leaders and represent the agency in meetings and committees.
- Serve as a resource for the care team, providing education on social and emotional factors related to patient care.
- Provide onsite crisis intervention and emotional support as needed.
- Coordinate behavioral and mental health referrals to appropriate specialists.
- Maintain accurate and timely records, including clinical and progress notes.
- Manage discharge planning upon completion of treatment.
- Coordinate with third-party payers.
- Maintain knowledge of transportation and other barriers to care.
- Triage patients over the phone.
- Conduct medication reconciliation.
- Provide in-service education and consultation for care teams regarding the social and emotional aspects of healthcare.
- Ability to work independently and collaboratively.
- Strong problem-solving, communication, and interviewing skills.
- Knowledge of community agencies and resources.
- Ability to communicate medical information clearly to professionals and patients.
- Skilled in organizing resources, setting priorities, and creative thinking.
- Proficient with personal computers, email, and related software.
- Bachelor’s Degree in Nursing and active RN license or Master of Social Work Degree from an accredited program.
- Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate (LCSWA) in good standing with the NCSWLB.
- Case management experience required.
