You will be a crucial part of the primary care team, offering coordination of care and care management services to patients at high risk. Our goal is to engage with these patients in a manner that enhances their quality of life and reduces hospitalizations. Our RN Care Managers actively contribute to developing care plans, ensuring seamless continuity of care within and outside the La Clinica services.

Role and Responsibilities:

• Working closely with the primary care team, providing clinical care interventions to patients of all ages
• Responsibilities - chronic disease management, health education, triaging patients and calls, and serving as a valuable clinical resource to the team

Qualifications:

• Passion for our mission of comprehensive patient wellness
• Current unrestricted Oregon RN license; ADN accepted, BSN preferred
• Two years of experience in an acute care or ambulatory care setting within the last eight years is preferred
• Epic electronic medical record experience preferred
• Spanish language proficiency is helpful but not required
