We're hiring an experienced Travel LMSW (Licensed Master Social Worker) for a 13-week Home Health assignment in Portland, Oregon! This is an excellent opportunity to work one-on-one with adult and geriatric patients while providing care coordination, psychosocial support, and community resource management.

Requirements:
- Minimum 2 years of LMSW experience
- At least 1 year of Skilled Home Health experience required
- First-time travelers welcome
- Active Oregon AND Washington Social Worker Licenses (Both required; pending licenses accepted)
- BLS Certification required
- OASIS experience required
- Epic experience preferred
- Must have reliable personal transportation
Key Responsibilities:
- Conduct comprehensive biopsychosocial assessments
- Develop and revise individualized care plans
- Coordinate care with interdisciplinary healthcare teams
- Provide patient and caregiver education
- Connect patients with community resources and support services
- Assist with Medicare, Medicaid, and other assistance programs
- Maintain accurate documentation in accordance with agency standards
- Participate in care conferences and care coordination activities

- Typical travel radius is up to 60 miles (approximately 60–90 minutes total driving per day).
- Facility may call off up to one shift per pay period.
