Job DescriptionCoordinates and facilitates the delivery of home care services in conjunction with hospital discharge planning through assessment of patient and family needs for appropriateness level, frequency and type of home care services to maximize anticipated patient outcomes.Education And CredentialsGraduate of accredited school of Registered Nursing. Current licensure to practice as a Registered Nurse in NYS. Current valid drivers license.ExperienceMinimum two (2) years of recent adult Home Health nursing experience. Demonstrated skills in customer sensitivity. Able to respond to inquiries and complaints appropriately with a focus on customer service. Demonstrated communication skills with physicians, discharge planners, and other professionals.
