Key Responsibilities
- Review and manage medication regimens for PACE participants
- Conduct medication reconcilliations and detailed chart reviews
- Identify, resolve, and prevent medication-related problems
- Particpate in interdisciplinary team (IDT) meetings and care planning
- Ensure compliance with CMS regulations, state pharmacy laws, and PACE standards
- Provide education to participants, caregivers, and healthcare team members
- Doctor of Pharmacy (PharmD) from an accredited program
- Active Pharmacist License (required)
- Active Controlled Substance License (required)
- 2 years of experience in geriatrics, long-term care (LTC), or related setting
- PACE experience preferred
- Willingness to travel
