Position: Admissions Nurse

Location: Honolulu, HI

Pay: $94,000 to $95,000/year

Experience: Previous experience in long-term care and community-based care strongly preferred.

Education: Registered Nurse License and a Bachelor’s degree from an accredited university or college.

Type: Full-time; Direct Hire

Schedule: Monday - Friday, 8:00am to 5:00pm

Kahu Malama Nurses is seeking an Admissions Nurse to join a growing and dynamic team!

Job Description

• Coordinate admissions for residents with challenging behaviors and financial/discharge planning risks.
• Collaborate with the interdisciplinary (ID) team to ensure smooth resident admissions and transitions.
• Assist with User-Defined Assessments (UDAs).
• Enter physician orders and discontinue completed orders as appropriate.
• Initiate and maintain individualized resident care plans.
• Accurately document and update resident information in PointClickCare.
• Ensure compliance with facility policies, procedures, and regulatory requirements.
  
  

Qualifications

• Proficiency with PointClickCare, including order entry and care plan management.
• Excellent organizational, communication, and critical thinking skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Must be able to read, write, speak, and understand English to communicate effectively.
• Demonstrates professionalism, flexibility, organization, assertiveness, and cooperation.
