Experience
- Previous experience working as an MDS Coordinator in a long-term care or skilled nursing facility.
- Minimum one (1) year of nursing experience in a long-term care or skilled nursing facility.
- Experience with care planning and quality improvement initiatives in skilled nursing facilities.
- Graduate of an accredited school of nursing.
- Current, valid Registered Nurse (RN) license in good standing in the State of Hawaii.
- Active RAC-CT certification through the American Association of Post-Acute Care Nursing (AAPACN).

Job Description
- Oversee the assessment and care planning process in the nursing department.
- Ensuring that MDS (Minimum Data Set) is accurate, complete, follows guidelines (for Medicare and Medicaid), and is submitted to the appropriate agency or provider.
- Coordinate resident admissions and initial services with members of the team.
- Provide general nursing care to residents on an as-needed basis.
- Working knowledge of the Minimum Data Set (MDS and the Resident Assessment Instrument (RAI).
- Proficiency with basic computer functions.
- Proof of annual tuberculosis clearance in accordance with state regulations.
- Valid CPR, First Aid, and AED certification.
- Knowledge of Resident Assessment Instrument (RAI) guidelines and Medicare/Medicaid reimbursement processes.
- Ability to solve problems under minimal supervision.
