Location: Honolulu, HI

Pay: This position has a fixed rate of $60/hour

Experience

• 1 year of Home Health experience
• Must have 6 months to 1 year of OASIS documentation experience

Schedule: Monday - Friday, 8:00am to 4:30pm (No on-call/weekends)

Kahu Malama Nurses is seeking a Travel Home Health RN to join a growing and dynamic team!

Job Description

• Conduct comprehensive patient assessments and manage all phases of home health care, including Admissions, Start of Care (SOC), Routine Visits, Recertifications, Resumption of Care (ROC), Transfers, Discharges, and Significant Change in Condition evaluations
• Provide ongoing case management and coordinate patient care plans to ensure quality outcomes and continuity of care
• Maintain accurate, timely, and compliant clinical documentation in accordance with home health documentation guidelines and regulatory standards
• Assess patient progress, identify changes in condition, and communicate updates to physicians and interdisciplinary care teams
• Manage discharge planning, including discharges related to safety concerns or patient non-compliance
• Deliver skilled nursing care across a variety of home health and patient care settings while promoting patient safety and independence
  
  

Position Requirements

• HI license required
• BLS required
• Driver's license with reliable transportation
