Position: Licensed Clinical Social Worker

Location: Hilo, HI

Pay: This position has a fixed rate of $42/hour

Experience

• Minimum of two (2) years of experience in a healthcare environment or equivalent setting.
• Experience with psychosocial palliative or hospice care, including assessments and care planning.
  
  

Education

• Master of Social Work (MSW) from an accredited institution required.
• Valid and current Hawaiʻi State Licensed Clinical Social Worker (LCSW) license required.
  
  

Type: Full-time; Contract

Schedule: Monday - Friday, 8:00am to 5:00pm

Kahu Malama Nurses is seeking a Licensed Clinical Social Worker to join a growing and dynamic team!

Job Description

• Provide comprehensive psychosocial assessments, counseling, care planning, and ongoing support for patients and families.
• Provide psychosocial palliative and/or hospice care, including support related to advanced illness and end-of-life needs.
• Deliver services through a combination of in-person visits, telehealth appointments, and clinic-based care.
• Advocate for patients and families while demonstrating empathy, emotional intelligence, and strong communication skills.
• Document patient services accurately and ensure documentation supports appropriate CPT coding and billing requirements.
• Connect patients and caregivers with community resources, including Medicare/Medicaid, financial assistance programs, and county, state, and private social service agencies.
• Other duties as assigned.
  
  

Position Requirements

• Knowledge of advanced illness management and related psychosocial issues.
• Working knowledge of insurance billing, CPT requirements, and applicable compliance standards.
• Strong clinical assessment, critical thinking, and documentation skills.
• Knowledge of community resources and social service programs.
• Proficiency with Microsoft Office, Outlook, internet-based applications, and general computer systems.
• Ability to use a personal vehicle and cell phone for work-related activities.
• Ability to work independently while maintaining effective collaboration with an interdisciplinary team.
