Admissions Coordinator (PRN)



General Purpose:

 

Responsible for maintaining targeted occupancy levels for the Judson health Center (JHC) by coordinating the admission process of residents to JHC through interfacing with hospitals, health care providers, private inquiries and other areas of Judson.

 

Essential Duties and Responsibilities:


  1. Maintain an up-to-date database for JHC census with new admissions, discharges, transfers and payor source changes.  Notifies appropriate personnel and affiliates of the current census status.
  2. Review/screen all resident referrals for appropriateness and Judson’s ability to meet the needs of the perspective resident.  Obtain input as needed from Director of Nursing and Administration, Administrator, and Therapy Department.
  3. Actively manage long- and short-term inquiries/referrals to establish personal relationships resulting in quality admissions.        Respond to and provide follow up on all JHC admission inquires in a timely manner.
  4. Communicate effectively with other departments and Judson Associates regarding impending new admissions into JHC.  Prepare and distribute admission information alert all appropriate Associates in advance of a confirmed admission.
  5. Assign a physician to follow the resident throughout their stay in JHC.
  6. Verify all insurance eligibility and complete skilled care updates for coverage
  7. Maintain a thorough knowledge of JHC’s products, services, acuity trends and physician relations.
  8. Maintain a thorough, up-to-date understanding of Medicare and Medicaid regulations.
  9. Notify environmental services and maintenance of vacated rooms to alert them to prepare the room for the new admission.
  10. Meet with new residents and/or families to review admission agreement information and obtain signatures.  Review insurance coverage as appropriate for level of care.
  11. Conduct tours for prospective resident/family members and respond knowledgeably to questions pertaining to Judson’s facilities and services.
  12. Interface with other health care staff in promoting a team approach to service planning and delivery for each JHC resident.
  13. In coordination with Judson marketing, provide visible avenues for informational development of JHC in the community.
  14. Generate and maintain census metrics and reports as directed.
  15. Comply with training requirements for individual training, and facilitates appropriate training for the department.
  16. Adhere to and promote compliance with requirements of Judson’s Anti-fraud and Anti-abuse programs.
  17. Additional responsibilities as assigned.

Minimum Qualifications, Education and Certifications:

  • Associate Degree in business or health related field
  • Bachelor’s degree preferred
  • Experience with Medicare and other insurance requirements preferred
  • Two or more years of experience in Admissions in a skilled nursing or long-term care environment, hospital, or other health related industry.
  • Excellent communication skills.
  • Excellent computer skills.
