Job Description
Description: The TCHATT Case Manager LMSW provides case management for children and adolescent patients, and their families. The TCHATT Case Manager provides case management for patients and family members, provides information, and makes appropriate referrals to resources within JPS Health Network and the community to include other social services facilities and/or agencies.
Typical Duties
- Provides case management for patients and family members of the TCHATT program. Conducts in-person and over the telephone consultations, provides information, and makes appropriate referrals to resources within the Network and the community to include other social services facilities and/or agencies.
- Gathers pertinent patient information from a variety resources (e.g. treatment sources, patients, family members etc.).
- Contributes towards the development, implementation, and evaluation of the patient’s treatment plan and acts as a liaison for family members, community agencies, and TCHATT staff and providers. Assists in the coordination of discharge planning activities and referrals.
- Collaborates with advanced standing licensed clinicians in reviewing discharge planning needs and alternatives for assigned patients and in accordance with policy and TCMHCC funding needs.
- Coordinates care with TCHATT staff, clinicians, and providers.
- Charts assigned patients per operating procedure and maintains other records as assigned.
- Contributes towards discharge planning, including assessment of needs, coordinating referrals, consulting with other service providers, and consultation with other community resources, as appropriate.
- Maintains current knowledge of community resources/information and issues affecting the populations served.
- Maintains competency relative to Social Work services for population served. Exhibits professional growth and development.
- Assumes, at times, responsibility for psycho-educational services or other responsibilities as assigned.
- Performs other related job duties as assigned.
Required Education and Experience:
- Master’s Degree in Social Work, Counseling or related field of study.
- 1-2 plus years’ of professional social work experience including internship work, performing any combination of: coordinating patient discharge and continuity of care planning; conducting initial screenings and evaluations of patients and families; providing or making referrals for individual, family, and group psychotherapy; and/or promoting patient navigation services.
- Licensed Master Social Worker (LMSW) and must maintain certification throughout employment.
- Basic Life Safety Certification (BLSC) required but may be obtained subsequent to hire.
