Home Health RN Weekend On-Call

Position Summary

The Weekend On-Call Registered Nurse provides after-hours and weekend nursing coverage to ensure continuity of patient care. The RN responds to urgent clinical needs, triages calls, provides direct patient care, and coordinates with the interdisciplinary team to deliver high-quality, compassionate care in accordance with organizational policies and state/federal regulations.

Key Responsibilities

• Serve as the primary on-call nurse during assigned on-call hours (typically Friday evening through Monday morning).

• Respond promptly to patient/family phone calls and determine appropriate interventions or visits.

• Conduct home or facility visits as necessary to assess patient condition, provide skilled nursing care, and implement physician orders.

• Perform scheduled visits totaling 8 points of productivity on both Saturday and Sunday, ensuring efficient workload management and timely completion of assigned visits.

• Provide emotional support and education to patients and families regarding care, medications, and disease processes.

• Collaborate with physicians, case managers, and other team members to ensure coordinated and effective patient care.

• Document all assessments, interventions, and communications accurately and on time in the electronic health record (EHR).

• Participate in handoff reports at the start and end of on-call shifts to ensure continuity of care.

• Maintain current knowledge of organizational policies, procedures, and professional nursing standards.

• Comply with infection control, safety, and confidentiality policies.

Qualifications

• Education: Graduate of an accredited School of Nursing; BSN preferred.

• Licensure: Current Registered Nurse (RN) license in the state of practice.

• Experience: Minimum 1–2 years of clinical nursing experience; home health, hospice, or acute care experience strongly preferred.

• Certifications: Current CPR/BLS certification required; additional certifications (ACLS, CHPN, etc.) preferred.

• Excellent communication, problem-solving, and organizational skills.

• Ability to work independently and manage multiple priorities.

• Comfort with electronic documentation systems and mobile technology.

Working Conditions

• On-call position requiring availability during scheduled hours, including nights, weekends, and holidays as assigned.

• May require travel to patient homes or facilities (reliable transportation and valid driver’s license required).

• Exposure to varying home environments and clinical conditions.

Salary : $90,000 - $95,000
