Housing Support Services – Case Manager (Per-Diem)

Help individuals at risk of or experiencing homelessness achieve housing stability and self-sufficiency. As a Housing Support Specialist, you’ll provide client-centered case management across Atlantic and Cape May Counties, connecting people to safe housing and critical support services.

Key Responsibilities:

• Conduct outreach in the community to engage and enroll individuals in services
• Perform assessments and develop individualized service plans
• Assist clients in securing and maintaining stable housing
• Provide relocation support, home safety planning, and follow-up care
• Connect clients with essential resources such as benefits, healthcare, and housing programs
• Maintain accurate and timely documentation in the Electronic Health Record (EHR)
• Track outcomes and ensure compliance with Medicaid and agency standards

Position Requirements:

• Bachelor’s degree in Social Work, Human Services, or a related field
• Experience in housing support, case management, or working with individuals experiencing homelessness preferred
• Familiarity with Medicaid documentation and community housing programs
• Valid NJ Driver’s License required
