What are the responsibilities and job description for the Case Manager - Community Support Services position at Jewish Family Service of Atlantic & Cape May Counties?
JOB SUMMARY
As a Case Manager in the Community Support Services (CSS) program, you will coordinate resources and services to meet individual client needs, actively engaging clients in their homes and in the field to support their treatment goals. This role requires maintaining a weekly average of 20 face-to-face hours with clients to ensure progress on their treatment plans. You will work as part of an interdisciplinary team, contributing to goal setting, consulting with the clinical team, and participating in program development and quality improvement initiatives.
Position Duties to include:
- Develop flexible, individualized service plans that address client needs and facilitate connections with community social services.
- Deliver supervised services, including individual, group, and family interventions, outreach case management, and advocacy for essential services based on client needs and goals.
- Provide direct transportation and support for client appointments and community activities.
- Offer skill-building and development training based on assessments of client progress.
- Organize and facilitate recreational and socialization activities that promote client well-being.
POSITION REQUIREMENTS:
- Master's Degree or Bachelor's Degree in Social Work or allied/related field or discipline (E.g. Psychology, Behavioral Health, Counseling, Psychiatric Rehabilitation) Note: General degrees are not sufficient.
- Valid NJ Driver License
