Jewish Family Service of Atlantic & Cape May Counties is looking for compassionate, motivated individuals to join our Homeless Services (PATH) team. This role provides community outreach, case management, and critical support to individuals experiencing or at risk of homelessness. Key Responsibilities:
- Conduct community outreach to individuals who are homeless or at risk, including participation in site-based locations and street outreach
- Complete assessments, intake forms, and service plans for eligible clients
- Provide information and referrals for individuals who do not meet service criteria
- Maintain accurate documentation of all outreach encounters and client services in the electronic health record
- Connect clients with housing programs, healthcare services, employment opportunities, mental health care, and substance use treatment
- Develop and maintain relationships with community partners to expand access to housing and supportive resources
- Attend community meetings to identify referrals and strengthen collaboration
- Provide ongoing support to assist clients in obtaining and maintaining housing, including searches, income support applications, and connection to government or community programs
- Manage a caseload and support clients in completing identified goals and determining case closure
Position Requirements:
- Bachelor’s degree in Social Work or related human services field (Psychology, Behavioral Health, Counseling, Psychiatric Rehabilitation)
- 0–2 years of case management experience
- Valid NJ Driver’s License required
