JOB SUMMARY: To facilitate and coordinate the provision of services to Infants/Toddlers and their families and to serve as a family advocate to ensure active participation in planning and appropriate service delivery. Bilingual in reading and writing in Spanish is required.

Essential Duties and responsibilities:          

  • Help families to understand the purpose of Early Intervention and the importance of the parent’s active participation
  • Work directly with families to access support and services for their child’s learning and growth.  Ensure that supports and services for families are provided according to required timelines.
  • Conduct initial and bi-annual routine-based interviews to access family priorities and concerns and explain the importance the family training model in Early Intervention.  Obtain pertinent information regarding child development.
  • Develop along with direct service providers, Individualized Family Service Plans (IFSPs) with families whose children are eligible for EI services.  Complete an Early Intervention Service Coordination Support Plan for each family.
  • Participate in team meetings with parents, caregivers, and providers to review outcomes and make recommendations
  • Work with families who may need interpretation and translation in order to fully participate in their child’s eligibility determination, service planning and transition.
  • Coordinate implementation of intervention services.  Assist families in identifying available service providers and facilitating communication with and between families and provider agency.  Meet with team members to develop and implement the IFSP.
  • Coordinate services provided to the child by on-going information sharing through participation in interagency meetings, telephone calls and/or face-to-face contact. Assist the child’s and family’s access to health care, community resources, services, and funding sources to help meet their needs.
  • Monitor the effectiveness of formal and informal support services and community resources provided to the individual, family/caregiver.  Monitor the timely delivery and effectiveness of EI services.
  • Problem-solve and resolve conflict with the family/caregiver and the provider of services. Assure the services and supports are on an ongoing basis and make changes as appropriate.
  • Schedule and participate in transition planning for eligible children exiting the EI (birth to 3-year-old) program within the mandated time frames.
  • Complete service notes in objective, person first language monthly for billing.
  • Maintain communication with agencies and programs serving children and families.
  • Maintain productivity standards as defined by his/her supervisor and agency.
  • Maintain punctuality and consistent attendance.
  • Perform other related duties as assigned

Qualifications:

EDUCATION/EXPERIENCE: BA, BSW or other bachelor’s degree required which includes or is supplemented by at least 12 college credits in Early Intervention, Early Childhood Special Education, Early Child Education, Child Development, Special Education, Family Counseling, Social Welfare, Psychology, or other comparable Social Science and 1 year of full-time or full-time equivalent experience working with or providing counseling to children, families, or individuals with special needs.  Computer skills, flexibility and excellent communication and organizational skills needed.  Need to be able to work well within a team model.  Bilingual ability is mandatory.

Computer Ability:  High level knowledge of Microsoft Office Suite, Word, PowerPoint presentations & table of organizations, Outlook, Excel spreadsheets, Familiarity with time and attendance software and development of internet surveys. Set up of on-line conference calls.  Ability to learn new software platforms when needed.

While performing the duties of this job, the employee is frequently required to sit and talk or hear. The employee is frequently required to stand; walk; use hands to finger, handle, or feel and reach with hands and arms. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
