You will be responsible for assisting the RN Case Manager in carrying out the plan of care for your assigned patients on an intermittent basis in their place of residence.

Brief overview of Responsibilities:
- Under the direction of a Registered Nurse, assists with the preparation, implementation, and ongoing evaluation of the patient care plan. Recognizes and understands the impact of social and economic challenges on patients and provides for the emotional and physical comfort and safety of patients and their families. Completes clinical and progress notes based on patient visits and performs ongoing assessments during each visit, documenting all relevant data. Observes, records, and reports patient symptoms, reactions, and changes to the appropriate person as needed. Assists the Registered Nurse with specialized procedures as directed under physician orders, participates in on-call duties according to policy, and completes other duties as assigned.
Qualifications:
- Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of hospice experience required.
- Graduate of an accredited nursing school.
- Must have a valid driver's license and a reliable car available for use during work hours.
