Job SummaryWe are seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care for a pediatric patient in a home health setting. In this role, you will work one-on-one with a child in their home environment, delivering personalized care, monitoring health status, and supporting the patient’s medical and developmental needs. This position offers the opportunity to build meaningful relationships with patients and families while providing high-quality, patient-centered care.The hours for this position is Mon-Fri 6a-6pResponsibilitiesProvide one-on-one skilled nursing care to a pediatric patient in the home setting.Monitor and document vital signs, respiratory status, and overall patient condition.Administer medications, treatments, and tube feedings as ordered by the physician.Assist with activities of daily living (ADLs) including hygiene, mobility, and comfort measures.Observe for changes in the patient’s condition and report concerns to the RN supervisor or physician.Maintain accurate and timely documentation according to agency policies and state regulations.Educate and support family members and caregivers regarding the patient’s care plan.Follow infection control and safety protocols within the home environment.QualificationsCurrent Licensed Practical Nurse (LPN) license in the state or a multistate compact license.Current BLS/CPR certification.Strong assessment, communication, and documentation skills.Ability to provide compassionate care to medically complex children and their families.
