Physical Therapist- Home Health (Full-time or PRN)
Are you a physical therapist committed to providing quality patient care? James River Home Health is growing and needs YOU! We are actively seeking a compassionate and highly committed Home Health Physical Therapist to join our growing team in the Spotsylvania area! You will be responsible for providing physical therapy to patients on an intermittent basis in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!
Availability: Full-time or PRN Opportunity Available
Territories: Mechanicsville, Henrico, Hanover, Richmond

A brief overview of Responsibilities:
Provides comprehensive patient assessments and develops individualized plans of care in collaboration with physicians and the interdisciplinary team. Delivers therapeutic interventions, evaluates equipment needs, ensures timely and accurate documentation, including OASIS requirements, supervises PTAs and HHAs as appropriate, participates in on-call rotation, and monitors patient outcomes to ensure high-quality care.
Qualifications:
Must be a graduate of a Physical Therapy school
Must be licensed or registered in Virginia.
One year of home health experience PLUS!
Must have current CPR Certification
