Brief Overview of Responsibilities for Hospice Clinical Manager:
Clinical Oversight
- Lead the coordination and delivery of hospice services across assigned territories. Supervise clinical staff, conduct regular chart audits, and drive QAPI initiatives to ensure high-quality, patient-centered care.
Staff Management
- Provide ongoing performance feedback, mentorship, and support to clinical team members. Facilitate orientation and training for new hires, and promote a collaborative, accountable work environment.
Compliance and Records
- Ensure timely and accurate documentation in accordance with Medicare, State, and ACHC standards. Monitor compliance trends and implement corrective actions as needed.
Direct Patient Care
- Deliver compassionate, skilled care in the home setting when needed to support coverage gaps, urgent needs, or complex cases.
Meetings and Supplies
- Actively participate in interdisciplinary team meetings, audits, and QAPI reviews. Monitor and maintain medical supply inventory to support uninterrupted patient care.
Professional Development
- Engage in ongoing professional growth through continuing education, workshops, and participation in industry events. Stay current with best practices in hospice care and leadership.
Qualifications:
- One year of clinical RN experience in home health or hospice
- One year of RN management experience
- Current Virginia State RN License
