Document all care per agency policy and report any changes in condition or deviations from the plan to the Case Manager. Take vital signs as directed, observe and report skin concerns, and assist with ambulation, transfers, and range of motion. Support the RN with specialized procedures under physician orders, participate in on-call duties, and perform other tasks as assigned.Qualifications:Certified Nurse Aide (CNA) with Virginia Department of Health Professions (Required)One year of experience as a CNA, Hospice Aide, or Home Health Aide.
