With 30 years of dedicated service, we are committed to delivering high-quality therapy services, including physical, occupational, speech, and aquatic therapy.Facility ManagerIrvine Neuro Rehabilitation is seeking an energetic and passionate leader of an 18-bed post-acute rehabilitation facility. We are looking for a dynamic person with a can-do outlook and motivated to make a positive impact with both residents we serve and direct caregiving team members!Oversee the daily operations of a residential facility for individuals with brain injuries, stroke recovery, spinal cord injuries, or other orthopedic challenges. Supervise and provide guidance to a team of caregivers and support staff.Adhere to MI AFC licensing requirements, CARF standards and company policy/practices. Ensure regulatory compliance.Lead Direct Care Team in providing high-quality, compassionate care and support to adult residents.Assign and maintain cleaning and facility up-keep duties.Take ownership of implementing QuickMar ADL tasks and narrative charting for each shift that aligns with resident goals. This includes physical, emotional, behavioral, and cognitive needs of each resident.Train direct care team to company expectations of duties and to individualized Plans of Care for each resident in assigned facility. Ensure physical, emotional and social well-being.Develop and foster regular and productive communication amongst team members and shift changes.Plan monthly staff meetings to encourage a sense of input, accountability and professional growth amongst team members.Collaborate with HR for disciplinary action discussions within 24 hours of occurrence. Offer immediate feedback, remediation and action plans to involved party(ies).Maintain integrity of emergency and maintenance binders. Conduct monthly inspections of documents for completion.Direct supervisor of facility team members. Structured 30- and 90-day check-ins, regular and ongoing mentorship/guidance and annual performance review completion.Partners with therapy team and nurse for any training needs identified.Work with internal case managers to re-assess residents needs and update care plans as necessary.Reviews daily documentation to ensure notes are clear, complete and concise. Re-educate and meet with team members 1:1 for additional training.Create shift schedules for assigned staff using both printed at-a glance monthly calendar and electronic software tools.Triage and find staffing solutions for call-offs and any instance of coverage needs.Flexibility to interface with all shifts throughout the course of a month.Coordinate and monitor medication management, supplies and training, in accordance with state regulations and facility policies. Partner with nurse for any noted medication issues or support.Pass medications in the event of staffing support need.Identify any Incident Reporting (IR) events to ensure accurate reporting, action and reducing future risk.Evaluate usage of supplies and ensure stock is available.Document assigned facility walk-throughs for any findings, maintenance needs or concerns/suggestions.Participate in educational opportunities and in-services.Engage with residents, their families and stakeholders to promote high level of customer services.Other duties as assignedAssociates or Bachelors Degree in related field3-5 years experience in a supervisory roleKnowledge of rehabilitative healthcare, nursing or therapy background a plus!Valid Drivers License with no more than three (3) pointsStrong organizational and communication skills, both written and verbal.Available primarily for Day shift but provides support, coverage, and training as needed to all shifts (first, second, third shifts).Frequent standing, walking, lifting, sitting, transferring carrying, reaching, pushing and pullingSufficient mobility to navigate all locations, including stairsAbility to respond quickly to routine and emergent patient care needs
