Interim HealthCare - New Hampshire is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Home Health team serving the Seacoast area of New Hampshire. In this rewarding role, you will provide personalized, home-based speech therapy services to patients experiencing communication, cognitive, or swallowing challenges due to illness, injury, or medical conditions.

Responsibilities
- Provide in-home speech therapy services to patients with communication, cognitive, and swallowing disorders
- Assess, diagnose, and develop individualized treatment plans based on patient needs and goals
- Evaluate patient progress and adjust therapy plans as appropriate
- Conduct therapeutic exercises related to speech, fluency, articulation, cognition, breathing, and swallowing
- Maintain accurate and timely patient documentation and clinical records
- Collaborate with interdisciplinary healthcare team members including RNs, LPNs, CNAs, HHAs, PTs, and OTs
- Educate patients and caregivers on treatment plans, safety practices, home exercises, and self-care strategies
- Ensure compliance with state and federal home health regulations and standards of care
Qualifications
- Master’s or Doctoral degree in Speech-Language Pathology
- Active New Hampshire SLP license in good standing
- Minimum of one (1) year of speech therapy experience preferred, ideally in home healthcare
- Current CPR certification
- Strong clinical judgment, communication, and critical thinking skills
- Compassionate, patient-focused approach to care
- Ability to work independently while collaborating effectively within a healthcare team
