We are currently seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled home health team serving Portsmouth and the surrounding Seacoast communities. In this role, you’ll provide direct patient care in a home setting where one-on-one attention allows you to truly connect with your patients and support their health and independence.

Responsibilities
- Provide skilled nursing care and treatments to patients in their homes
- Monitor patient conditions and report changes to the supervising RN or physician
- Administer medications and treatments as prescribed
- Maintain accurate and timely clinical documentation and patient records
- Assist with patient assessments and individualized care planning
- Educate patients and family members on medications, treatments, disease management, and safety practices
- Collaborate with physicians, nurses, therapists, and caregivers to support patient goals
- Ensure compliance with state and federal home health regulations and company standards
Qualifications
- Current Licensed Practical Nurse (LPN) license in the State of New Hampshire
- Minimum of two (2) years of skilled nursing or acute care experience preferred
- Current CPR Certification required
- Reliable transportation and valid driver’s license
- Strong communication and clinical assessment skills
- Compassionate, dependable, and patient-focused mindset
- Home healthcare experience preferred, but not required
