We’re searching for a Chinese, Cantonese and/or Mandarin speaking Care Manager I who is passionate about helping older adults thrive safely in their homes and communities. About the Role
This position provides care coordination and case management support for a meaningful panel of participants enrolled in our On Lok PACE program. You’ll work closely with an assigned Medical Social Worker and the participant’s full Interdisciplinary Team (IDT), ensuring that each individual’s physical, emotional, and social needs are not just met, but honored.
Key Responsibilities
- Establish and maintain a supportive care management relationship with participants and their formal/informal support systems, always offering dignity and respect.
- Meet with clients as needed and collaborate with the Medical Social Worker to monitor and support participants’ biopsychosocial needs.
- Identify participant needs, advocate on their behalf, coordinate services, and ensure timely referrals to both internal and external providers.
- Assist with transitions of care and partner with IDT members to arrange and implement necessary services.
- Coordinate required documentation and communication for participants traveling outside the service area.
- Facilitate discharges to other programs or community settings as appropriate.
- Document all case management activity and maintain required paperwork according to departmental standards.
- Communicate participant concerns to the Medical Social Worker and/or IDT members promptly and effectively.
- Attend and actively participate in IDT meetings, program discussions, and problem-solving efforts.
- Contribute to continuous quality improvement and promote a culture of collaboration.
- Utilize supervision appropriately, keeping lines of communication open and transparent.
- Incorporate NASW ethical standards into all interactions and decisions.
- Apply all PACE regulatory requirements, licensing standards, and IOA policies.
- Stay current on case management principles, gerontology, family systems, and other relevant areas.
- Perform all other reasonably related duties as assigned.
Required Qualifications
- BA or BS in Social Work or closely related field (required).
- At least one year of professional experience working with a frail older adult population.
- At least one year of experience in a case management role, ideally with older adults or functionally impaired adults.
Background & Experience
- Experience with the medical and psychosocial needs of frail elderly and functionally impaired adults.
- Strong communication and presentation skills for working with participants, caregivers, and interdisciplinary health teams.
- Demonstrated case management experience within the community health care delivery system.
- Highly detail-oriented, with strong problem-solving abilities and the capacity to prioritize multiple tasks.
- Computer literacy preferred.
