What You'll Be Doing:
- Carrying out treatment plans developed by the OT to support patient rehab goals
- Assisting patients with exercises, mobility training, and adaptive techniques
- Educating patients and caregivers on home-based strategies for continued progress
- Documenting patient responses and progress clearly and timely
- Collaborating with the care team to ensure continuity and quality of care
- Traveling between patient homes throughout the assigned region
What You’ll Need:
- Active California COTA license
- Minimum 1 year of experience in a home health setting
- Strong communication and organizational skills
- Valid driver’s license and dependable transportation
- Current CPR certification and TB clearance
Preferred:
- Experience in hospice and/or geriatric populations
