Position Overview
We are seeking a dedicated and compassionate professional to provide mental health support services to individuals ages 18 and older. This role is responsible for the development, implementation, and coordination of individualized service plans in alignment with program standards. The ideal candidate will be client-centered, trauma-informed, and skilled in crisis intervention, ensuring individuals receive high-quality care and support during behavioral health crises.
Key Responsibilities
- Provide on-call, rapid crisis response to individuals experiencing behavioral health emergencies.
- Engage individuals in environments where they feel most comfortable, facilitating stabilization and resolution whenever possible.
- Deliver services in community-based settings, including homes, schools, workplaces, and other social environments.
- Ensure the safety of individuals and others while minimizing law enforcement involvement, emergency room utilization, and avoidable hospitalization.
- Prevent escalation of symptoms that could result in out-of-home placement or disruption of the individual’s living environment.
- Refer, link, and coordinate access to behavioral health services and supports across the continuum of care, including pre-admission screenings as appropriate.
- Collaborate with behavioral health providers and community partners to support continuity of care.
- Provide recovery-oriented, trauma-informed, and developmentally appropriate services that integrate Zero Suicide / Suicide Safer Care principles.
- Deliver culturally responsive care that honors each individual’s lived experience, preferences, and personal goals.
- Utilize de-escalation strategies and strong interpersonal communication skills to resolve conflict and maintain client engagement.
- Maintain availability to provide services during day, evening, overnight, weekend, and holiday shifts.
- Conduct ongoing safety assessments and monitoring for clients and others involved.
- Recommend appropriate step-down services following mobile crisis intervention and assist with discharge planning.
- Complete all required documentation, progress notes, timesheets, and reports in a timely and accurate manner.
- Participate in all required meetings, trainings, supervision, and team collaboration efforts.
Skills & Competencies
- Strong knowledge of DSM-5 diagnoses
- Case management and individualized service plan development
- Crisis intervention and de-escalation techniques
- Excellent oral and written communication skills
- Ability to maintain confidentiality and uphold ethical standards
- Familiarity with community resources and behavioral health systems

Minimum Qualifications
- Bachelor’s degree in Health and Human Services or a related field (e.g., Social Work, Psychology, Counseling, Sociology, Criminal Justice, Human Services Counseling, Special Education, Gerontology, Psychiatric Rehabilitation, or Vocational Rehabilitation) from an accredited college or university.
