Description:
Provide mental health support services to individuals ages 18 and older. This role is responsible for the development and implementation of individualized service plans in accordance with program guidelines and client needs.
Responsibilities:
· Provide direct services to clients, including linkage to resources, advocacy, support, and psychoeducation as needed.
· Prepare written individualized service plans, incorporating input from clients and involved service providers, while clearly identifying problem areas, needs, strategies, and service objectives.
· Respond to crisis situations within twenty-four (24) hours.
· Link clients with external programs and community resources, including health services, recreational activities, childcare services, financial assistance, employment resources, AA/NA groups, transportation services, and other supportive resources.
· Collaborate and coordinate with other service providers and professionals, including attorneys, social workers, and probation/parole officers.
· Complete all progress notes and quarterly reports in a timely and accurate manner.
· Recommend appropriate services based on client needs and clinical presentation.
· Ensure clients are informed of their rights and protections.
· Maintain weekly contact with referring agencies.
· Conduct discharge planning in accordance with program guidelines.
· Attend all required meetings and trainings.
· Submit accurate timesheets in a timely manner.
· Advise the Clinical Director on the status of clinical services, staffing needs, and program challenges, while collaborating on effective solutions.

Minimum Qualifications:
· Must hold, at minimum, a Bachelor’s degree in Health and Human Services, including but not limited to social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, vocational rehabilitation, criminal justice, human services counseling, or a related field from an accredited college or university.
· Experience providing mental health services and case management to individuals living with mental illness is preferred.
· Must be credentialed by the Virginia Department of Health Professions as a Qualified Mental Health Professional-Adult (QMHP-A) or QMHP-Adult/Trainee (QMHP-A/T).
This position also requires a valid driver’s license, automobile and telecommunications availability, excellent verbal and written skills; counseling and social work theories and techniques; and community resources. Strong writing and case conceptualization skills and reference to a methodological. The successful completion of a criminal history or background investigation is required.
