This position provides services at the TCRHCC main campus. Pharmacist is responsible for providing a comprehensive pharmacy and medication management service to patients and ensures a consistent level of service in accordance to TCRHCC policies. Direct patient care and services are provided to neo-natal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients. This position requires working rotational shifts: day, evening, nights, weekends, and Holidays. The position provides the opportunity for career advancement through tier levels of Pharmacist to Advanced Practice Pharmacist as determined by pharmacy experience. Advanced Practice Pharmacist serves as a clinical pharmacist for direct clinical pharmacy services for inpatient and outpatient departments. In addition to the responsibilities of Advanced Practice Pharmacist I, Advanced Practice Pharmacist II is responsible for all specialized pharmacy–based clinics and providing an ongoing analysis of clinical services to develop programs that commensurate with the highest quality of care. Responsibilities also include the administration of policies and procedures and directing programs and activities in accordance to TCRHCC's mission, vision, and values. Duties: The following is a shortened version of the full duties for the Pharmacist position at TCRHCC: Conduct outpatient pharmacy clinics, assess treatment plans, and monitor patient outcomes. Review medical records for safe and effective drug therapy; consult with medical and nursing staff. Verify and input chemotherapy treatment plans for oncology patients. Counsel patients and families on medication use and compliance. Participate in disaster drills and respond to actual emergency events. Train and educate pharmacy staff, students, and healthcare professionals. Maintain and expand pharmacological knowledge through continuing education. Provide drug information and consult on pharmacokinetics across age groups. Attend inpatient rounds, respond to emergencies, and manage therapeutic drug monitoring. Ensure proper documentation and update patient records. Use PPE appropriately and follow infection control protocols. Complete training for cleaning and decontamination procedures. Gain competency in pharmacy software systems. Supervise pharmacy technicians and ensure efficient service delivery. Meet performance benchmarks and KPIs. Perform other duties as assigned by Pharmacy leadership.
