Mental Health Targeted Case Managers help consumers gain access to services in their community to meet their mental health needs. Case Managers help coordinate services to support areas such as, but not limited to, mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation. Case Managers play a critical role in assisting someone with a mental health disability live independently in their community.
Duties/responsibilities include but are not limited to:
- Provide services in the four core case management components – Assessment, Planning, Referral and Linkage, and Monitoring and Coordination of services.
- Conduct Assessments
- Coordinate with a mental health professional to assure the completion of a mental health assessment of client needs and develop an individual treatment plan.
- Develop and update the client’s individualized plan (ICSP). Coordinate, monitor and evaluate services that are outlined in the plan for appropriateness, progress and continued service need.
- Discuss, research, resource and facilitate successful housing transitions for individuals
- Meet with the consumer as outlined in the treatment plan and program policies.
- Keep necessary case notes documenting contact with the client, their family and other contacts.
- Maintain client files.
- Coordinate with consumer’s team
- Advocate with Consumer
- Crisis planning and intervention.
- Manage a caseload of around 30 consumers.
Education:
- Bachelor’s degree in social work, psychology, sociology or related field
- At least one year experience in health care or human services field that included assessment and service planning

Wages:
- $42,000 - $47,000 annually, starting wage based on experience
Frequently Asked Questions
Hours- Schedule is determined by staff and consumers. Traditional hours are 8-4:30 but are flexible. Meetings with children may require hours outside of school hours. Staff can create their own schedule.
Mileage reimbursement- Mileage is reimbursed at .58 cents per mile
Working remotely- Yes, position is remote with occasional travel.
Travel locations - we cover an increasing number of counties throughout the state, travel to visit with consumers could include a variety of locations.

Evaluations - Evaluations occur every September for the year. All employees have monthly 1:1s with their supervisor to consistently evaluate performance. In Sept the evaluation is a monthly 1:1 but encompassing the whole year.
