SCHEDULE

Full Time: Monday – Friday, day hours, 40 hours a week

HOW YOU’LL MAKE AN IMPACT

Behavioral Health Targeted Case Management (TCM) provides home and community-based mental health services to adults with severe and persistent mental illness (SPMI), and children with severe emotional disturbance (SED) and their families.

• Establish relationships with assigned clients and supports and meets with each client as planned.
• Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
• Be familiar with implementation of health integration, including care coordination agency wide.
• Maintain current knowledge of the mental health service system with the ability to location information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.
• At a minimum meet the monthly required contact for each client on caseload; document all services provided accurately, completely and within required deadlines.

WHAT YOU’LL NEED

• Qualify as a Behavioral Health Practitioner (BHP)

• Clear DHS background study
Valid Driver’s license and acceptable driving record.

WHAT WILL SET YOU APART

• Experience working with individuals living with SPMI/SED or co-occurring disorders.
• Knowledge of community resources and experience connecting clients with needed support.

SCHEDULE

Full Time: Monday-Friday ability to choose your hours between 7:00 AM – 6:00 PM
