Job duties:
- Assumes responsibility and accountability for the practice of professional nursing in accordance with Agency policy and standards of practice to ensure effective and appropriate home care services.
- Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
- Communicates effectively and in accordance with Agency requirements.
- Assess’ the continuing needs of the patient. Performs initial and ongoing patient’s assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services, as needed.
- Participates with team of RNs, LPNs and or Home Health Aides to provide effective and quality home care services.
- Promotes personal safety and a safe environment for patients and coworkers.
- Performs job duties in accordance with Agency policies, procedures, and professional and community standards.

Qualifications:
- Graduate of an accredited school of professional nursing.
- Current license as a Registered Nurse.
- Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
- Ability to deal tactfully with patients and the community.
- Demonstrated organizational and time management skills.
