Responsibilities:
- Provide personal care assistance including bathing, grooming, dressing, and hygiene support
- Assist clients with mobility, transfers, and ambulation as needed
- Monitor and report changes in patient condition to supervising nursing staff
- Support clients with light housekeeping, meal preparation, and daily living activities
- Document care provided accurately and in a timely manner
- Build trusting, respectful relationships with clients and their families
- Follow individualized care plans and adhere to home health agency protocols
Requirements:
- Current CNA, HHA, or home health aide certification preferred
- Prior experience in home health, personal care, or a related caregiving setting
- Availability to work weekends on a PRN (as-needed) basis
- Reliable transportation to travel to client homes in the Nineveh, IN area
- Strong communication skills and a compassionate, patient-centered attitude
- Ability to work independently with minimal supervision
- Must pass a background check and meet all state compliance requirements
