Responsibilities:
- Provide personal care assistance including bathing, grooming, dressing, and hygiene support
- Assist patients with mobility, transfers, and ambulation as needed
- Monitor and document patient health status and report changes to supervising clinical staff
- Support patients with meal preparation and nutritional needs
- Perform light housekeeping and maintain a safe, clean patient environment
- Deliver compassionate companionship and emotional support to patients and families
- Follow individualized care plans and adhere to home health agency protocols
- Current CNA certification or Home Health Aide (HHA) credentials in the state of Indiana
- Previous experience in home health, long-term care, or a similar patient care setting preferred
- Strong communication skills and ability to work independently in a home environment
- Reliable transportation and valid driver's license required
- Ability to pass a background check and drug screening
- Compassionate, patient-centered attitude with a commitment to quality care
- Flexibility to work PRN shifts as needed
