Independence Home Health LLC is looking for a dedicated and compassionate Full-Time RN Case Manager to join our growing team! Position Details:

• Full-time, Monday–Friday
• Report to the office at 8:00 AM to pick up the company vehicle for scheduled visits
• Complete patient visits and case management responsibilities as scheduled
• Work collaboratively with patients, families, caregivers, and the clinical team to ensure quality care and continuity of services
  
  
  

Qualifications:

• Current, valid Registered Nurse (RN) license
• Experience in home health preferred
• At least one to three years of case management experience preferred
• Strong communication, organizational, and critical-thinking skills
• Compassionate, dependable, and patient-focused
