Independence Association seeks a full time, energetic Adult Case Manager to join our dynamic and growing case management team. As an Adult Case Manager, you will play a key role in assisting individuals with intellectual disabilities or autism to access resources to live full and inclusive lives. You will work closely with both the individual and their identified support group, and you will provide thorough assessments of strengths and needs, comprehensive planning of services, advocacy, outreach and resource coordination unique to every person. Quick synopsis:

  • Assess, plan, implement, and evaluate each client's needs
  • Create, implement, and oversee treatment plans
  • Assist client's in making well informed decisions and assist with learning self-advocacy
  • Educate and support each client
  • Timely, organized documentation
  • Other duties as needed/assigned

Requirements for Adult Case Manager:

  • Bachelor's degree (required)
  • One year experience working with adult individuals with intellectual disabilities and /or autism (required)
  • Travel required (must have valid driver’s license)
  • Outgoing, self-directed, great written and oral communication skills, team player
  • Comfortable advocating for individuals with various levels of disability in a variety of settings

Job Type: Full-time

Pay: $47,000.00 - $52,000.00 per year
