JOB SUMMARY:
As a Team Support Specialist, you’ll provide dynamic, person-centered direct care that includes hands-on personal support, positive role modeling, and skill-building to promote independence and growth. Team Support Specialists provide flexible coverage across multiple home settings, supporting care teams where needed while ensuring consistency, safety, dignity, and a high quality of life for the individuals served.
ESSENTIAL FUNCTIONS:
- Provide direct care to individuals in multiple homes
- Oversee and/or assist with personal care and hygiene routines
- Plan and implement community integration activities
- Assist with daily tasks such as meal prep, shopping, and housekeeping
- Maintain thorough documentation, daily logs, and follow each Individual Plan (IP)
- Provide a safe environment by adhering to emergency procedures
REQUIRED SKILLS:
- Excellent communication, written and organization skills.
- Ability to teach/mentor others.
- Ability to assist in training new employees
- Ability to use computer programs such as Therap, Station MD, and Paycom.
KEY REQUIREMENTS:
- 18 years of age
- High school diploma or equivalent
- Valid driver's license (ability to obtain class E license within 14 days of hire)
- Proof of vehicle insurance
- Successful completion of background checks and reference verification
- Work flexible schedules, including evenings, nights, weekends, and holidays.
- Ability to stand or sit for several hours and bend, stoop
