As a Program Manager for Community Living, you will oversee all aspects of assigned residential homes, driving exceptional, person-centered support for individuals with intellectual and developmental disabilities. Essential Functions:
- Oversee assigned Community Living homes to ensure quality services, effective communication, and positive outcomes.
- Coordinate healthcare, medical appointments, and annual assessments; participate in individual planning.
- Hire, train, supervise, and evaluate direct care staff; manage scheduling and payroll within budget.
- Ensure timely and accurate documentation, reporting, and monthly progress notes.
- Maintain safe, clean homes and coordinate maintenance and landlord communication.
- Communicate professionally with internal/external stakeholders and coordinate with agencies to prevent service disruptions.
- Monitor financial stability of individuals in coordination with accounting.
- Ensure compliance with program standards (DMH, CARF).
- Provide on-call and direct care coverage as needed
Skills Required to Perform the Duties of the Job:
- Excellent communication, writing, and organization skills
- Ability to use computer programs such as Microsoft, Excel, Outlook, StationMD, Paycom, and Therap.
- Skill in making decisions independently in accordance with program and agency standards.
KEY REQUIREMENTS:
- 18 years of age
- High school diploma or equivalent (Preferred: B.S. or B.S. in related field and/or 3 years of experience with individuals with ID/DD.)
- Supervisory experience preferred
- Valid driver's license (ability to obtain class E license within 14 days of hire)
- Proof of vehicle insurance
- Successful completion of background checks and reference verification
- Work flexible schedules, including evenings, nights, weekends, and holidays.
- Ability to stand or sit for several hours and bend, stoop
Salary : $50,000 - $65,000
