About the Illinois Migrant Council (IMC)
The Illinois Migrant Council (IMC) is a statewide, community-based nonprofit organization dedicated to promoting employment, education, health, housing, and other opportunities for migrant and seasonal farmworkers, immigrants, and rural communities to achieve economic self-sufficiency.
IMC is a proud partner Illinois Department of Human Services (IDHS) — a collaborative effort with IPHA, the Illinois Public Health Association, and other community-based organizations to support families across Illinois. The program specifically is the SNAP Connect Program.
Position Summary
Under the supervision of the Program Coordinator, the Outreach Specialist / Community Health Worker provides direct support to families through case management, outreach, and resource coordination. This role focuses on helping clients access essential public benefits, community services, and programs that enhance their health, well-being, and economic stability.
The ideal candidate is a community-driven professional who values equity, inclusion, and advocacy, and who can work effectively with individuals from diverse linguistic and cultural backgrounds.
Key Responsibilities
- Conduct outreach, intake, and eligibility screening for IMC programs.
- Outreach activities will include McHenry, Jo Davis, Carroll, Lee, and Putnam Counties
- Provide individualized case management and referrals for services related to housing, healthcare, education, employment, and public benefits.
- Collaborate with regional partners, agencies, and local organizations to coordinate services and strengthen client support networks.
- Maintain accurate client records, progress notes, and performance data in accordance with program guidelines.
- Participate in community events, workshops, and cross-training to promote IMC programs and strengthen partnerships.
- Assist clients with navigation of public systems and advocacy for immigrant rights and civic engagement.
Qualifications
- Associate’s degree or two years or more of relevant experience.
- Strong communication and interpersonal skills, with the ability to engage clients one-on-one and in small groups.
- Bilingual in Spanish and English preferred.
- Basic computer literacy (Microsoft Office, Salesforce, and internet usage).
- Valid Illinois driver’s license, auto insurance, and reliable vehicle required.
