IJN Health Systems,LLC is a Skilled Home Health Agency in Chadds Ford, PA is looking for Physical Therapist to join our strong compassionate to provide rehabilitation services .We are located on 225 Wilmington W Chestder Pike STE 200. Our ideal candidate is a self-starter, motivated, and engaged and dependable.
Physical Therapist (PT) – Home Care, PRN.
Position Summary:
- Provide services for the purpose of treating impairments, functional limitations, disabilities or changes in physical function and health status for patients normally performed in their home.
- Plan, organize and implement physical therapy services within the confines of state licensing law to individuals, families and groups in accordance with agency policies, procedures, processes and agency code of ethics.
Qualifications / Requirements
- Bachelor's degree, Master level or DPT, which meets the requirements of an approved physical therapy program, and is licensed through examination by the State Board of Physical Therapy.
- Graduate of a Physical Therapy education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or APTA.
- One year of Physical Therapy experience required.
- One year of home health experience preferred.
- Competency in computer skills.
- CPR certified
- Pass a background check
- Provide two (2) professional references
- Able to travel to cases as assigned
- Ability to work independently and manage time effectively
- Meets the requirements of federal and/or state criminal and abuse background checks
- Must have access to dependable transportation, auto insurance coverage
