Key Responsibilities

• Evaluate and treat residents/patients with speech, hearing, swallowing, and cognitive disabilities in accordance with state and federal guidelines.
• Develop and implement individualized treatment plans based on evaluations and physician directives.
• Provide direct therapy services to improve communication, swallowing, and cognitive functions.
• Administer, score, and interpret specialized hearing and speech assessments.
• Educate residents, families, and caregivers on therapy goals and strategies for practicing new skills.
• Maintain accurate clinical documentation, including evaluations, notes, and treatment summaries, adhering to organizational and regulatory standards.
• Collaborate with interdisciplinary team members to deliver comprehensive patient care in a skilled nursing facility setting.
• Ensure patient privacy and confidentiality in compliance with HIPAA.
• Participate in developing and adjusting treatment plans with clear short- and long-term goals.

Required Qualifications

• Valid Speech-Language Pathologist license in the state of Oregon.
• Proven experience in speech therapy, particularly with complex cases.
• Ability to evaluate and treat a diverse caseload independently.
• Strong knowledge of speech therapy techniques and clinical best practices.
• Experience working with geriatric populations and in skilled nursing facilities.
• Excellent documentation and communication skills.

Preferred Experience / Skills

• Specialized experience in swallowing therapy.
• Familiarity with rehabilitative therapies for cognitive and communication impairments.
• Ability to develop custom tools and treatments tailored to individual patient needs.
