Key Responsibilities
- Evaluate and treat residents/patients with speech, hearing, swallowing, and cognitive disabilities in accordance with state and federal guidelines.
- Develop and implement individualized treatment plans based on evaluations and physician directives.
- Provide direct therapy services to improve communication, swallowing, and cognitive functions.
- Administer, score, and interpret specialized hearing and speech assessments.
- Educate residents, families, and caregivers on therapy goals and strategies for practicing new skills.
- Maintain accurate clinical documentation, including evaluations, notes, and treatment summaries, adhering to organizational and regulatory standards.
- Collaborate with interdisciplinary team members to deliver comprehensive patient care in a skilled nursing facility setting.
- Ensure patient privacy and confidentiality in compliance with HIPAA.
- Participate in developing and adjusting treatment plans with clear short- and long-term goals.
Required Qualifications
- Valid Speech-Language Pathologist license in the state of Oregon.
- Proven experience in speech therapy, particularly with complex cases.
- Ability to evaluate and treat a diverse caseload independently.
- Strong knowledge of speech therapy techniques and clinical best practices.
- Experience working with geriatric populations and in skilled nursing facilities.
- Excellent documentation and communication skills.
Preferred Experience / Skills
- Specialized experience in swallowing therapy.
- Familiarity with rehabilitative therapies for cognitive and communication impairments.
- Ability to develop custom tools and treatments tailored to individual patient needs.
